Transistional Services
Transitional care refers to care provided following hospitilization or stay in a rehabilitation center. We understand that leaving the hospital or a skilled nursing facility to go home could lead to uncertanity and social problems for individuals. NexGen Home Health can professionally deliver care that can help you reduce undesired outcome such as re-hospitalization, potential medication errors or falls. We offer services and the expertise to assist you in making a safe and healthy recovery at home.
